Thomas T. Teel, D.D.S.
Dr. Teel graduated from the Indiana University School of Dentistry in 1988 and completed a General Practice Residency at the Medical College of Ohio in 1989, where he received advanced training in all aspects of general dentistry. He has completed advanced cosmetic curriculum at the Las Vegas Institute for Advanced Dental Studies and at the Pacific Aesthetic Continuum in San Francisco. Less than one percent of all dentists have completed this level of aesthetic training.
Over the past several years, Dr. Teel has shared his knowledge of cosmetic and restorative dentistry with dentists from across America. He has lectured at numerous meetings and conferences, and has directed live-patient hands-on programs. As a consultant to several dental manufacturers, he has helped test and develop new materials and techniques for the dental industry.
Recently, Dr. Teel was named one of America's Best Dentists by his peers, as researched and published by Woodward/White, Inc. The publication, The Best Dentists in America, is based on an exhaustive peer review study, where the voting is anonymous and listings cannot be purchased for inclusion. Dr. Teel is the only practicing general dentist in Allen County to be listed.
Dr. Teel is a member of the American Dental Association, Isaac Knapp District Dental Society, Academy of General Dentistry, and the American Academy of Cosmetic Dentistry. He and his wife, Patrice, enjoy outdoor activities including biking, hiking, skiing and golfing. They are the proud parents of two children, Maria and Ethan.

Thomas M. Gilbert,
D.D.S.
Dr. Gilbert, a Fort Wayne native, graduated from Homestead High School in 1987. He received his undergraduate degree from Indiana University in 1991 and graduated from the Indiana University School of Dentistry in 1995. Dr. Gilbert has attended advanced training courses in various aspects of general dentistry. He has a special interest in the field of implant dentistry. He has also completed advanced training in complex restorative dentistry at the prestigious Dawson Center for Advanced Dental Studies. He also volunteers his time and expertise treating patients at the Matthew 25 Health & Dental Clinic.
Dr. Gilbert is a member of the American Dental Association, Indiana Dental Association, Isaac Knapp District Dental Society, the Academy of General Dentistry, Xi Psi Phi Dental Fraternity, International Congress of Oral Implantologists, and American College of Oral Implantology. He is also a faculty member at IPFW where he teaches in the Dental Hygiene program. In his free time, Dr. Gilbert enjoys tennis, fishing, running and golf, and spending time with his wife, Amy, and sons, Mitchell and Kyle.

Catherine N. Periolat,
D.D.S.
Dr. Periolat was raised in Valparaiso, Indiana and graduated from Purdue University in 1992 with a degree in Genetics. She received her Doctorate of Dental Surgery from Indiana University School of Dentistry in 1996, and for the following three years operated a private practice in Fort Wayne, later she was a staff dentist at the Neighborhood Heath Clinic. She is interested in all aspects of restorative dentistry and enjoys providing excellent care and service to our patients. She continues her education by regularly attending advanced courses to stay current on the latest advances in dentistry.
Dr. Periolat has served on Isaac Knapp District Dental Society Board of Directors and as Chairperson of the Give Kids a Smile Program for the Indiana State Dental Association. She was recognized by her peers for her contributions when she received the "Rising Star Award" from the Indiana Dental Association. She is a member of the American Dental Association, Indiana Dental Association and the Isaac Knapp District Dental Association. Dr. Periolat enjoys spending time with her husband, Ben, and their two children, Ellis and Grace. She admits to being a busy person, and enjoys golf, reading, knitting and crocheting, watching movies and basket making.
